Connecting Older Adults
with Options for Living Well


AgeOptions helps older adults, people with disabilities and caregivers in suburban Cook County, Illinois, live their lives to the fullest by empowering them with information and resources. We partner with a network of nonprofit community-based organizations and can connect you or someone you care for with programs and services in your community, such as:

  • Telephone assistance to answer questions and link callers to the resources they need

  • Assistance in identifying and applying for benefits

  • Nutritious meals

  • Intervention and education to prevent fraud, abuse and neglect

  • Caregiver support

  • In-home care
  • Adult day services

  • Help making Medicare choices 

  • Workshops for people with ongoing health conditions

  • Help moving out of a nursing home

  • Employment and volunteering opportunities

AgeOptions Interactive Referral Page

Click here to find your local Care Coordination Unit (CCU), using your home address. It will provide you with contact information for the CCU. Contacting the CCU will initiate the process to determine eligibility and connect you to appropriate meal services, adult protective services, and caregiver services.

Fraud Alerts

Illinois Senior Medicare Patrol (SMP) sends email Fraud Alerts every other Monday. Click here for the alerts, which contain fraud-related news articles, information about current scams in Illinois or other parts of the U.S. and useful health care or fraud-related resources.

Elder Abuse, Neglect & Financial Exploitation Intervention & Prevention

To report suspected elder abuse, neglect or financial exploitation, call the Illinois Department on Aging’s Elder Abuse Hotline at (866)800-1409. Reports may be made anonymously. Click here for more information or call AgeOptions at (708)383-0258.
